Have You Moved or Changed Your Address?

It is very important to keep your mailing address current.  Please take a moment to check your last pay stub to make sure your address is correct.  If not, please complete a “Change of Status Form” and mail it back to Human Resources and notify payroll immediately.

Lost Your W2 Form?

If you lost your W-2 form and need a replacement, you may mail or fax a written, signed request for a replacement to Rose Lee in the Finance Department.  Our address is P.O. Box 1549, Suffolk, VA 23439-1549 and our fax number is 757-925-6763.  Or you may make the request via email to Rose Lee through the Suffolk Public Schools address listing.  Replacement W-2s will be mailed the following Monday after your request is received.  Link here for the form.
