CTE Testing Consent Form Available Here

Any student who is under the age of 18 and currently enrolled in a Career and Technical Education (CTE) course offering a Certiport exam must have a consent form signed before he or she can be registered and take the Certiport industry credentialing exam.

This agreement is solely between the school division and the parent/legal guardian giving consent for the student to test.
