School Athletic Physicals

Physical form must be filled out (except section 3) prior to the 14th.

Forms are available in the Main office, Athletic Director's office, with Ms. Smith and online

Ms. Smith must complete the prospective athlete’s height, weight, and blood pressure.

On the day of the physical, the student must bring a completed physical form (with all required information and physical preliminaries) to the school with a fee of $10.00. This fee MUST be cash or money order ONLY. No checks will be accepted.

The doors will close at 6:00 and no students will be screened unless they are there before 6.

Students can start picking up the physicals and seeing Ms. Smith as soon as possible.
