Enrollment or New Student Registration

Suffolk Public Schools would like to welcome all new families. If you and your family are new to our school community, please review the information below to prepare for the enrollment process.

ince schools are currently closed, parents should call the school to schedule a time to bring in all of the documents to complete the enrollment process.

To register your student, you will need the following:

  • Student’s birth certificate
  • A photo ID for parent/guardian
  • Proof of residency – this can be an electric, gas, or water bill showing usage within the last 30 days. If you do not have any of these items, contact the Office of Student Services for assistance at 757-925-6750.
  • School Health Information Form – the school physical form is available online at the Virginia Department of Education’s website. This form should be completed by your child’s physician and should include proof of adequate age-appropriate immunization.
  • Custody or court orders – if applicable
  • Foster care information form – if applicable
  • Individual Education Plan (IEP) or 504 – if applicable


The following documents can be completed online or downloaded. Once you have all of the completed documents ready, you can call the school to schedule an appointment to complete enrollment. If you need assistance or have any questions, please contact the Office of Student Services at 757-925-6750

Below are some questions for parents to ask their pediatrician's office when making an appointment based on work flow recommendations to doctors' offices from the American Academy of Pediatrics:

Is there a way to complete necessary forms and payments electronically or by phone prior to the visit?  Do you have a patient portal for this information?

Is telehealth an option if all immunizations are up to date?

Do you have specific exam rooms designated for sick and well visits? (If your pediatrician has more than one office location, ask if certain locations are designated sick/well sites)

Do you have a separate entrance for sick and well visits?

Are you scheduling well visits in the morning and sick visits in the afternoon?

Do you have a text/phone call option for waiting so the child and parent can remain in the car until it is their turn to be seen?
