Suffolk Public Schools will NOW OFFER Covid-19 Vaccine Clinics to include ages 5-11.

Walk-ins are welcome. However, pre-registration is strongly encouraged. The vaccine clinics are open to students ages 5-11 years old - with an accompanying parent/legal guardian. Vaccines may also be available to those ages 12+. We will have a total of three clinics.

If your child is not enrolled at the location you choose to attend from those listed above, we ask that you not arrive before 4:30 pm to allow for afternoon dismissal traffic to clear.

 

Please print and complete the consent form located below. Copies are also available at each school. Bring the completed consent form with you to the vaccine clinic.

The vaccine being offered is the Pfizer vaccine. The Emergency Use Authorization (EUA) for ages 5-11 can be found HERE. This is a two-dose vaccine series. The 2nd dose will be offered 21 days following dose one. An additional flyer will be posted as a reminder of dose two.
